Need a permission slip? You can use one of ours!
We do not require permission slips in order for your students to attend our programs. However, if you would like to use one of the below permission slips for any requirements of your school/educational organization, feel free to use these. They are free for you to use.
We do not publish photos or videos of anyone’s face without their permission. If we request to share a photo or video of someone, we will ask them or a guardian to sign one of these release forms.
